Cefixime trihydrate
Indications/Uses
Otitis, media, bronchitis, UTI, pharyngitis, bronchitis. Taxim-O Otitis media & other URTI where the causative organism is known or suspected to be resistant to other commonly used antibiotics, or where treatment failure may carry significant risk; cystitis, cystourethritis, uncomplicated pyelonephritis. Taxim-O 400 UTI caused by E. coli & Proteus mirabilis; uncomplicated gonorrhea caused by N. gonorrhoeae; otitis media caused by H. influenzae (β-lactamase +ve & -ve strains), Moraxella (Branhamella) catarrhalis, (most of which are β-lactamase +ve) & Strep pyogenes; tonsillitis, caused by Strep pyogenes. Taxim-O Kids Gonorrhea.
Dosage/Direction for Use
Taxim-O Adult & childn >10 yr 200-400 mg daily according to the severity of infection as a single dose or in 2 divided doses for 7-14 days. Renal impairment (CrCl <20 mL/min) & patients who are maintained on chronic ambulatory peritoneal dialysis or haemodialysis 200 mg once daily. Taxim-O 400 Adult 200-400 mg as a single dose or in 2 divided doses. Uncomplicated cervical/urethral gonococcal infections 400 mg as a single oral dose. Childn 8 mg/kg/day as a single daily dose or in 2 divided doses. Childn weighing >50 kg or >12 yr Adult dose. Renal impairment (CrCl 21-60 mL/min) or patients who are on renal hemodialysis 75% of the standard dose at the standard dosing interval (300 mg daily), CrCl <20 mL/min or patients who are on continuous ambulatory peritoneal dialysis 1/2 the standard dosage at the standard dosing interval (200 mg daily.) Taxim-O Kids Childn 5-10 yr 200 mg daily, 1-4 yr 100 mg daily, 6 mth-1 yr 75 mg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to cefixime, cephalosporins or penicillins. Taxim-O 400 Pseudomembranous colitis.
Special Precautions
Taxim-O Discontinue treatment when severe cutaneous adverse reactions occur. Drug-induced hemolytic anemia. Renal impairment. Premature or newborn infant. Taxim-O 400 Patients on dialysis. History of GI disease particularly colitis. Risk of fall in prothrombin activity in patients w/ renal/hepatic impairment, poor nutritional state or those receiving protracted course of antimicrobial therapy.
Adverse Reactions
Eosinophilia; agranulocytosis; hemolytic anemia; leukopenia, thrombocytopenia, neutropenia; abdominal pain, dyspepsia, nausea, vomiting, diarrhea; pseudomembranous colitis; increased ALT/AST, blood bilirubin, blood urea, blood creatinine; jaundice; dizziness; anaphylactic/anaphylactoid reactions; skin rashes; serum sickness-like reactions; pruritus, genital pruritus; headache; drug fever; SJS, TEN, angioedema, urticaria, vaginitis; erythema multiforme; face edema. Taxim-O Granulocytopenia, thrombocytosis; flatulence; dyspnea; renal failure including tubulointerstitial nephritis; DRESS, arthralgia, pyrexia. Taxim-O 400 Loose stools; hepatitis; acute renal failure; seizures; hyperbilirubinemia; candidiasis. Taxim-O Kids bleeding complications related to hypoprothrombinemia &/or platelet dysfunction.
Drug Interactions
Increased prothrombin time & enhance effects of the anticoagulants w/ anticoagulants. False +ve reaction for glucose in the urine w/ Benedict's or Fehling's soln or w/ Cu sulfate test tab; Coombs' test. Taxim-O 400 Elevates the levels of carbamazepine.
